About you
Full name • Email • Mobile phone • Town / ZIP code
Preferred location: Wayne / Haskell / Sparta / Clifton. Which other locations could you work at?
Position(s): Counter & customer service / Food runner & dining room / Kitchen & prep / Pizza maker / Dishwasher / Delivery driver / Shift leader / Open to available positions.
Part-time or full-time? Desired hours per week? Earliest start date? Are you seeking year-round, school-year, summer-only, or seasonal work? If temporary, what is your last available date?
Your weekly availability
For each day, Monday through Sunday: earliest start time, latest end time, or unavailable. List the hours you can consistently maintain, including travel time after school or activities.
How many shifts can you commit to weekly? Can you work Friday evenings, Saturdays, Sundays, and school breaks? Describe any limits. Are there specific dates you already cannot work?
Your schedule over the next six months
Will your availability change during the next six months? Yes / No / Not sure.
List any school, sports, clubs, theater, lessons, another job, or other commitments that affect your working hours. You only need to share scheduling details, not private personal information.
For every commitment: start date, end date, practice/meeting days and times, game/performance days, travel requirements, and whether the schedule is confirmed or tentative.
What will your work availability be during that period? Which days and times will remain available? If your schedule is not confirmed yet, when will you receive it?
Example: “Softball starts March 1 through May 30. Practice Monday–Thursday until 5:30; some Saturday games. Available Tuesday/Thursday after 6 and Sunday all day. Game schedule arrives February 15.”
List planned vacations, camps, trips, exams, and school transitions, with dates. Is there an upcoming change that means you could work now but substantially reduce hours or leave shortly afterward?
How much notice can you give before a known schedule change? When can we discuss your availability if clarification is needed?
Experience & working in a pizzeria
Is this your first job? If yes, no problem—tell us about relevant school, volunteer, team, or customer-service experience.
Most recent employers (up to two): business, position, employment dates, duties, reason for leaving, and whether we may contact them. Do not provide sensitive information about anyone else.
Which tasks have you done? Taking orders / POS & payments / Customer service / Food prep / Pizza making / Cooking / Dishwashing / Cleaning / Delivery / Supervising. Describe experience and any food-safety certification.
Tell us about a time you followed through on a commitment. What would you do if you realized you could not make a scheduled shift? How would you handle an upset customer or a mistake during a rush?
Can you reliably arrive for the shifts you list? Are you legally authorized to work in the United States? Any position-specific requirements will be reviewed separately before hiring.
